PLEASE NOTE: You must first attempt to resolve your attendance record issue with the course section instructor prior to submitting a formal attendance grievance. Do not submit this form unless you have done so.

Grievance forms must be submitted prior to the end of the term following the term in which the attendance records in question were recorded. Students who fail to begin the resolution process by this deadline forfeit their right to submit attendance grievances.

I was present in-person (on campus) in the class meeting, but the instructor erroneously entered my attendance as "present, web conference" in Self-Service for the date of the class meeting. I was present in-person (on campus) in the class meeting, but the instructor did not enter an attendance record in Self-Service for the date of the class meeting.I was present in-person (on campus) in the class meeting, but the instructor recorded that I was "absent" in Self-Service for the date of the class meeting.I attended a class meeting or participated in the online course later than the Last Date of Attendance (LDA) that was entered by the instructor.
