Processing Steps:

To add a certificate program, this form will be forwarded to the student’s academic advisor. If the request is approved, the advisor will add the certificate program to the student’s academic record.


The academic advisor will respond to the student’s request via email within 20 working days of receiving the petition. Students may contact their academic advisor directly with any follow-up questions.
